Job Overview – Case Manager- IDD:
Join our client’s team in Philadelphia, PA, as a Case Manager for individuals with Intellectual and Developmental Disabilities (IDD). In this role, you will facilitate individual plan development, coordinate services, and support community integration, ensuring compliance with local, state, and federal mandates.

Compensation: $24/hour
Location: Philadelphia, PA
Schedule: 9:00 AM to 5:00 PM, Monday to Friday


Responsibilities as the Case Manager:
  • Service Coordination: Facilitate the development of individual plans, locate providers, and coordinate services.
  • Advocacy & Support: Advocate for individuals, using natural supports and enhancing individual/family strengths.
  • Needs Assessment: Identify and address vocational, social, emotional, psychological, and other needs using a person-centered process.
  • Documentation: Maintain accurate service notes and records in compliance with HCSIS and other requirements.
  • Plan Development: Develop and update Individual Support Plans (ISP) and fiscal year renewal plans, ensuring all relevant parties are involved.
  • Budget Management: Develop budgets for needed services, assist individuals in identifying providers, and monitor resource utilization.
  • Monitoring & Reporting: Conduct monthly and/or quarterly monitoring, ensuring services are delivered as specified in the ISP.
  • Incident Management: Promote health and safety, review incident reports, and document occurrences in non-provider settings.
Qualifications for the Case Manager:
  • Education: Bachelor's or Master's degree with at least 12 credits in social sciences.
  • Experience: 1+ year of related experience or equivalent combination of education and experience.
  • Skills: Strong language, mathematical, and computer skills, including proficiency in HCSIS, MS Word, and MS Outlook.
  • Licenses: Valid driver's license, proof of insurance, and vehicle registration required for mileage reimbursement.
  • Travel: Ability to travel regionally 50%-70% of the time depending on assigned caseload.
