Job Overview - Case Manager Bilingual - Spanish:
Join our compassionate team as a Case Manager in Transitional Housing. You'll play a crucial role in assisting individuals experiencing homelessness, some with mental illness and/or substance use disorders, transition to permanent housing.
Location: Brooklyn, NY Compensation: $22 - $24/hr Schedule: Monday to Friday from 9 am to 5 pm.
Responsibilities as the Case Manager Bilingual - Spanish:
Manage a caseload of approximately 15 clients, offering direct support and conducting assessments to aid in housing and recovery goals.
Conduct psychosocial evaluations.
Connect clients to psychiatric, medical, and substance use treatment facilities as necessary.
Assist with obtaining identification, public benefits, and other income sources.
Provide weekly client meetings for needs assessment, harm reduction strategies, and service plan development.
Prepare documentation for housing applications.
Counsel clients in the process of obtaining permanent housing.
Maintain record-keeping standards compliant with agency and governmental requirements.
Crisis intervention when necessary.
Escort clients to appointments as needed and assist with medication self-administration after proper training.
Qualifications for the Case Manager Bilingual - Spanish:
Bachelor’s Degree or equivalent work experience.
Two years of experience working with homeless adults and substance abuse issues is preferred.
Proficient in Microsoft Word, Excel, and the ability to learn new programs.
Excellent writing and communication skills.
Bilingual in Spanish/English is required.
