Role Overview:
A housing services organization is seeking a Case Manager to empower formerly homeless individuals to remain successfully housed through tailored guidance and community linkage. Leveraging partnership talents and knowledge of support programs, the ideal candidate will conduct home visits, provide benefits assistance, coordinate group activities, and promote life improvement outcomes through person-centered planning.
Compensation: $24.75 / HR Location: New York City
Responsibilities:
• Carrying a caseload of approximately 15 clients
• Performing assessments and developing care plans
• Connecting clients to medical/psych/SUB services
• Helping obtain IDs, income sources, entitlements
• Building trusting relationships through home visits
• Liaising with local providers and programs
• Responding sensitively in crisis scenarios
• Maintaining timely documentation
Qualifications:
• Bachelor's degree or 2+ years relevant experience
• Background in substance use and motivational interviewing preferred
• Valid driver's license and clean record
• Strong technology skills and communication abilities
• Passion for uplifting vulnerable groups
