Job Description

Role Overview:
We are seeking a dedicated Case Manager to provide comprehensive case management services to a caseload of approximately 22 clients. The Case Manager will conduct assessments, develop service plans, and perform home visits and fieldwork to support clients' independence and well-being.

Compensation: Temp: $24.00 / HR | Perm: $40,000.00 - $45,000.00
Location: Bronx, New York
Schedule: Monday - Friday, 9 am - 5 pm

Responsibilities:
•    Ensure clients receive appropriate and effective case management
•    Conduct comprehensive assessments and develop service plans with short/long-term goals
•    Perform face-to-face contacts (minimum 1 home visit per month, 1 contact per week)
•    Assist clients in accessing medical care, medication adherence, daily living skills, nutrition, recreation, employment and other supportive services
•    Review and reassess case plans quarterly to align with clients' current needs
•    Provide crisis intervention and ongoing monitoring of clients' capacity for independent living
•    Coordinate with internal and external providers, including OMH personnel
•    Maintain accurate documentation of all client contacts and services provided
•    Participate in weekly supervision, training workshops, and discharge planning
•    Assist clients with housing transitions, financial management, and advocacy
•    Perform other duties as assigned by supervisor or Program Director

Qualifications:
•    Bachelor's degree required
•    1-2 years of fieldwork experience in OMH direct services
•    Knowledge of case management principles and OMH Scattered Site Program regulations
•    Excellent organizational, communication, and problem-solving skills
•    Ability to work effectively with individuals with severe mental illness
•    Proficiency in Microsoft Office (Word, Excel)
•    Bilingual in Spanish preferred
•    Valid driver's license and ability to travel for home visits
