Job Overview - Care Delivery Supervisor (RN):
Join our client's team in New York, NY, as a Care Delivery Field Supervisor, where you will oversee clinical services provided at participants’ homes. You will ensure the delivery of high-quality care by supervising nursing staff and collaborating with departments to support day-to-day operations.
Compensation: $110,000 - $120,000/year Location: New York, NY
Setting: Field-based, covering all of Manhattan
Schedule: 8:30 AM to 5:30 PM, Monday to Friday
Responsibilities as the Care Delivery Supervisor (RN):
Supervision: Oversee and guide CHNs, HHAs, and other staff in delivering home health care services.
Assessment & Care Planning: Evaluate participants' healthcare needs and collaborate with the IDT to develop personalized care plans.
Field Visits: Conduct joint visits with CHNs for supervision and competency checks.
Documentation: Ensure accurate and timely documentation in compliance with regulations.
Education & Training: Assist in staff orientation and provide necessary education to participants and aides.
Regulatory Compliance: Maintain knowledge of health regulations and ensure compliance.
Qualifications for the Care Delivery Supervisor (RN):
Education: Degree from an NLN-accredited nursing school (RN, BSN preferred, or 5 years of experience).
Experience: 1+ year in Home Care or Community Health Nursing (experience with elderly populations preferred).
Licensing: Active RN license in New York State; must be authorized to practice in New York.
