Job Overview - Care Delivery Manager, RN:
We're seeking a dedicated Care Delivery Manager, RN, to join our team in Flushing, Queens. Bilingual in Korean and passionate about elevating nursing standards? This role is for you! Oversee daily operations, ensure seamless care delivery, and enhance patient satisfaction and outcomes.

Location: Queens, NY
Compensation: $115,000 - $118,000/year
Schedule: 8:30 AM to 5:00 PM, Monday to Friday


Responsibilities as the Care Delivery Manager, RN:
  • Care Coordination: Create and implement personalized care plans that enhance patient satisfaction and outcomes, facilitate Interdisciplinary Team (IDT) meetings, and manage follow-ups during non-operational hours.
  • Clinical Oversight: Generate and analyze clinical delivery reports, ensure precise assessments, and investigate and document all incidents thoroughly.
  • Team Leadership: Balance staffing and resources, ensure comprehensive team coverage, and collaborate with HR for staff recruitment, training, and performance evaluations.
  • Patient Interaction: Manage care across multiple sites, coordinate long-term care services, and maintain effective communication with all relevant parties to ensure cohesive care delivery.
  • Compliance & Quality Improvement: Maintain high documentation standards, propose service enhancements, and ensure full regulatory compliance at all levels.
  • Education & Support: Offer continuous staff development, provide clinical education, and assess the performance and competencies of direct reports.
Qualifications for the Care Delivery Manager, RN:
  • Education: Degree from an accredited nursing school (RN, BSN required); completion of an accredited Nursing Program; BA degree preferred; NYSED-approved infection control coursework.
  • Experience: At least 2 years in a management role within a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, or nursing home setting; customer service experience required; managed long-term care insurance experience is a plus.
  • Licensing: Current active and unrestricted RN license in New York State.
  • Language: Must be bilingual in Korean.
  • Skills: Excellent leadership, communication, and organizational skills; ability to handle complex challenges and prioritize tasks effectively.
