Job Overview - Care Coordinator:
Looking for a fulfilling role in care coordination? Join our Program in Staten Island, NY, as a Care Coordinator. You will be crucial in managing client care and ensuring they receive the necessary support and services.
Location: Staten Island, NY
Compensation: $25/hour
Schedule: 9 AM to 5 PM, Monday to Friday
Responsibilities as the Care Coordinator:
Provide care coordination for clients, ensuring access to necessary services like medical care, behavioral health services, and social support.
Advocate for client services via phone and in-person.
Conduct home visits and follow-up to monitor client status.
Participate in case conferences and training sessions.
Maintain relationships with service providers.
Keep accurate and timely documentation in client charts and records.
Qualifications for the Care Coordinator:
Bachelor's Degree or High School Diploma with 2 years of care management experience.
Experience working with homeless and HIV populations.
Bilingual skills in Spanish preferred.
Exceptional communication skills, both oral and written.
Clean valid driver's license required.
