Position Overview:
A Health Homes program is seeking a Care Coordinator to serve a caseload of clients in Staten Island. Key responsibilities include coordinating care, conducting home visits, advocating for services, participating in case conferences, maintaining client records, building provider relations, and ensuring access to medical/social services.
Compensation: $25.00/HR
Location: Staten Island, New York
Schedule: Monday - Friday, 9 AM - 5 PM
Responsibilities:
• Provide care coordination activities for assigned clients
• Ensure access to needed medical, behavioral and social services
• Advocate for clients by phone and through in-person visits
• Perform home visits to monitor client status
• Participate in case conferences and training
• Maintain relationships with healthcare providers
• Document timely and accurate client notes and records
• Attend supervision meetings with Care Manager
Qualifications:
• Bachelor’s degree or HS diploma with two years case management experience
• Experience with homeless/HIV groups required
• Bilingual Spanish preferred
• Exceptional communication skills, verbal and written
• Valid NY driver’s license
