Job Overview – Bilingual Spanish Family Support Specialist:
Join our client as a Bilingual Spanish Family Support Specialist in Brooklyn, NY, where you will provide critical home-based support and case management services to families in the Transitions Brooklyn prevention program. Your efforts will help ensure child safety, family stability, and access to essential resources, while promoting a strength-based and trauma-informed approach.

Compensation: $24/hour
Location: Brooklyn, NY
Schedule: 35 Hours/Week


Responsibilities as the Bilingual Spanish Family Support Specialist:
  • Family Visits: Collaborate with Case Workers/Social Workers to conduct frequent home and office visits to monitor child safety and family stability.
  • Community Resource Access: Establish and maintain linkages to community resources, assisting families in accessing medical care, mental health services, housing, childcare, and public benefits.
  • Accompaniment and Advocacy: Accompany participants to educational, medical, and treatment appointments, and provide support at community and government agencies.
  • Administrative Support: Manage administrative tasks, including consent forms, medical/educational record updates, and case tracking to meet program deadlines.
  • Documentation: Maintain accurate and timely progress notes, update databases (Connections and PROMIS), and summarize school and treatment reports.
Qualifications for the Bilingual Spanish Family Support Specialist:
  • Language: Bilingual in English and Spanish required.
  • Education: High School diploma required.
  • Experience: At least 1 year of experience working with children or families in a community-based setting.
  • Skills: Proficiency in Microsoft Office (Outlook, Excel, and Word) required (familiarity with Connections and PROMIS databases a plus).
  • Attributes: Sensitivity to diverse cultures and backgrounds, strong advocacy skills, and a commitment to youth and family development.
  • Flexibility: Ability to travel throughout Brooklyn for home visits and work up to two evenings per week, as needed.
