Job Overview - Bilingual Case Manager:
Take on a vital role as a Bilingual Case Manager in New York, NY, providing crucial support to clients. You’ll assess needs, coordinate services, visit clients, and manage case notes to ensure comprehensive care.
Compensation: $24/hour Location: New York, NY
Schedule: 9:00 AM to 5:00 PM, Monday to Friday
Responsibilities as the Bilingual Case Manager:
Community Visits: Assess referred allegations and determine the need for APS services through client visits in the community.
Case Documentation: Complete case notes, interactions, and progress notes to maintain accurate client records.
Reassessment: Conduct monthly reassessments of client needs and revise case plans accordingly.
Case Decisions: Render decisions based on APS eligibility criteria and address client inquiries.
Service Coordination: Submit referrals for services to ensure client needs are met.
Qualifications for the Bilingual Case Manager:
Language Skills: Bilingual in Spanish, Russian, or Creole required.
Skills: Computer proficiency, good writing skills, and strong time management.
Experience: Ability to work with diverse adults who are mentally and/or physically impaired.
Knowledge: Familiarity with the five boroughs and public transportation (travel reimbursement provided).
