Job Overview - Bilingual Case Manager:
We are seeking a motivated Bilingual Case Manager to join our Adult Protective Services team in our New York office. This hybrid role is crucial for assessing and supporting clients in the community, ensuring their safety and well-being.
Location: New York, NY
Compensation: $24/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday (Hybrid)
Responsibilities of the Bilingual Case Manager:
Community Assessments: Visit clients to evaluate allegations and determine the need for services.
Documentation: Maintain accurate case notes, interactions, and progress notes.
Client Reassessment: Conduct monthly reassessments and update case plans accordingly.
Case Management: Make case decisions based on APS eligibility criteria.
Client Support: Address client inquiries and submit referrals to meet their needs.
Qualifications for the Bilingual Case Manager:
Language Skills: Bilingual in Spanish, Russian, or Creole.
Technical Skills: Proficient in computer use.
Communication Skills: Strong writing skills.
Interpersonal Skills: Ability to work with diverse adults with mental and/or physical impairments.
Knowledge: Familiarity with the 5 boroughs and public transportation.
Time Management: Excellent time management skills.
