Job Overview – Bilingual Case Manager (Spanish):
Embark on a new healthcare career journey as a Bilingual Case Manager! This role involves providing support services to help individuals remain housed and address various needs such as benefits, daily living activities, substance abuse, and mental health services. The Case Manager conducts home visits, links clients with local services, and coordinates group activities.

Compensation: $24/hour
Location: Brooklyn, NY
Schedule: 9:00 AM to 5:00 PM, Sunday to Thursday


Responsibilities as the Bilingual Case Manager:
  • Direct Services: Provide support to a caseload of approximately 15 clients, including home visits, assessments, and benefits assistance.
  • Client Assessments: Conduct initial and ongoing assessments, including biannual and psychosocial evaluations.
  • Crisis Intervention: Provide crisis intervention and connect clients to psychiatric, medical, and substance use treatment services.
  • Community Connection: Link clients to job training programs, recreational activities, and community services.
  • Benefits Assistance: Assist clients in obtaining identification, public benefits, and other income sources.
  • Service Planning: Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans.
  • Documentation: Maintain records in compliance with agency standards and those of City and State agencies.
  • Crisis Management: Intervene in crisis situations and provide assistance with self-administration of medication as per policies.
Qualifications for the Bilingual Case Manager:
  • Education: Bachelor’s Degree or equivalent work experience.
  • Experience: 2+ years of experience working with homeless adults and substance abuse issues preferred.
  • Skills: Proficiency in harm reduction and motivational interviewing techniques preferred.
  • Licensing: Valid NY driver’s license.
  • Technical Proficiency: Excellent computer skills, including Microsoft Word and Excel.
  • Language: Bilingual in Spanish and English preferred.
