Job Description

Job Overview - Bilingual Case Manager:
Join our team as a Bilingual Case Manager and make a significant impact in the lives of people experiencing homelessness. Located in Brooklyn, NY, you'll work closely with individuals to support their transition to permanent housing and recovery.

Location: Brooklyn, NY
Compensation: $24/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday


Responsibilities of the Bilingual Case Manager Include:
  • Client Services: Provide direct case management to a caseload of approximately 15 clients, including conducting assessments and offering benefits assistance.
  • Psychosocial Evaluations: Conduct evaluations to understand clients' needs and connect them to psychiatric, medical, and substance use treatment services.
  • Benefits and Documentation: Assist clients in obtaining identification, public benefits, and other income sources, and prepare documentation for housing applications.
  • Client Meetings: Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans.
  • Crisis Intervention: Intervene in crisis situations and escort clients to appointments as needed.
  • Medication Assistance: Upon training, assist clients with the self-administration of medication according to organizational policies.
  • Record Keeping: Maintain records in compliance with agency standards and those of City and State agencies.
Qualifications for the Bilingual Case Manager Include:
  • Experience: Two years of experience working with homeless adults and substance abuse issues preferred. Experience with harm reduction and motivational interviewing techniques is strongly preferred.
  • Bilingual: Must be bilingual in Spanish and English.
  • Education: Bachelor’s Degree or equivalent work experience.
  • Skills: Excellent computer skills, including proficiency in Microsoft Word and Excel. Strong writing and communication skills are essential.
