Role Overview:
Our client, a leading child services agency in Brooklyn, is seeking a Licensed Practical Nurse to evaluate and coordinate integrated whole-person care for adolescents in foster placements. Leveraging strong nursing fundamentals and passion for community work, the ideal candidate will manage care transitions across teams of providers, conduct home visits, provide counseling, administer treatments, recommend local resources, and promote self-determined, trauma-informed futures through collaborative relationship building.
Compensation: $40.00 / HR Location: Brooklyn, New York Schedule: Monday - Friday from 10 am to 6 pm
Responsibilities:
• Identifying and addressing health needs
• Establishing primary and specialty care homes
• Liaising between various youth providers and facilities
• Delivering injections, medications and assessments
• Charting timely and accurate progress notes
• Educating youths and caregivers on health topics
• Responding sensitively to behaviors and crises
Qualifications:
• Licensed Practical Nurse certification
• Recent pediatric experience ideal
• Valid CPR skills and clear driving record
• Strong communication and documentation
• Comfort with multidisciplinary coordination
• Passion for guiding vulnerable groups
