Role Overview:
The registered nurse (RN) assumes a pivotal role as the lead for medical and physical health within the Housing Provider Team, tasked with evaluating the current service needs of class members as they transition to their supported apartments.
Position Type: Temp to Perm - 6 Month Temp Period Compensation:$65.00 / HR Location: Queens, New York Schedule: Monday - Friday, 9 am - 5 pm

Responsibilities:
Collaborate on clinical summaries for client transitions
Provide expertise to determine eligibility
Perform health assessments
Make care recommendations
Formulate transition recommendations
Maintain documentation
Conduct home visits
Provide crisis intervention
Participate in team meetings
Coordinate medical and medication services
Qualifications:
NY Registered Nurse License
Bachelor's in Nursing
Graduate of Accredited Nursing School
Certification in geriatric nursing preferred
Experience in assessments
5+ years with serious mental illness
Knowledge of medication management
Organized and efficient
Communicates effectively
Compassionate and culturally sensitive
