Job Overview – Case Manager (Supportive Housing)
Compensation: $26/hour
Location: Brooklyn, NY
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
TAG MedStaffing is hiring a Case Manager (Supportive Housing) in Brooklyn, NY for our client, supporting housing stability and direct case management. In this role, you will connect program participants to medical, behavioral health, and community resources while delivering person-centered, trauma-informed services that promote long-term stability and community integration.
Responsibilities as the Case Manager (Supportive Housing):
Case Management & Service Planning: Manage an assigned caseload and develop, implement, and monitor individualized service plans in partnership with program participants.
Crisis Intervention & Client Support: Provide problem-solving support and crisis intervention to address mental health, substance use, and daily living challenges.
Client Advocacy & Care Coordination: Advocate for program participants while coordinating with therapeutic providers, community organizations, and internal teams to ensure comprehensive care.
Appointments & Home Visits: Conduct home visits and accompany participants to medical, psychiatric, housing, and public benefit appointments as needed to support continuity of care.
Documentation & Compliance: Complete timely and accurate service documentation in accordance with agency, funder, and regulatory standards.
Qualifications for the Case Manager (Supportive Housing):
Education: Bachelor’s degree in Social Work or a related human services field required.
Experience: 2+ years of experience providing case management or supportive services to individuals with mental illness, substance use disorders, and/or a history of homelessness.
Industry Knowledge: Experience with group facilitation, tenant engagement, referrals, community outreach, or housing-focused services strongly preferred.
Technical Skills: Proficiency in Microsoft Office Suite, including Word, Excel, and Outlook, with the ability to maintain accurate electronic records.
Skills & Attributes: Compassionate, organized professional with strong communication skills, sound judgment, and the ability to manage a caseload in a fast-paced environment.
Language Skills: Bilingual in English and Spanish preferred.
