Job Overview – Physical Therapist:
Compensation: $60/hour
Location: New York, NY
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
Our client is hiring a Physical Therapist in New York, NY. In this role, you will conduct patient assessments, design and administer treatment plans, and deliver therapy both on-site and in field settings such as homes and nursing facilities. You will collaborate with a multidisciplinary team, document progress accurately, and support care planning to improve mobility, reduce pain, and enhance patient outcomes.
Responsibilities as the Physical Therapist:
Patient Assessment & Documentation: Conduct evaluations, progress notes, and discharge summaries, maintaining accurate records in EMR systems.
Therapeutic Treatment: Provide physical therapy sessions at the clinic, in patient homes, and in nursing facilities.
Care Planning: Collaborate with the interdisciplinary team to design and implement individualized treatment plans.
Safety & Compliance: Ensure a safe environment by following health, safety, and departmental protocols.
Discharge & Coordination: Oversee discharge planning, coordinate equipment needs, and consult with physicians and healthcare professionals.
Qualifications for the Physical Therapist:
Education: Successful completion of a Physical Therapy program from an APTA-accredited institution or equivalent required.
Experience: Minimum of 1 year of experience as a Physical Therapist required, with geriatric care experience strongly preferred.
Technical Skills: Proficiency in EMR systems such as Care Compass preferred.
Industry Knowledge: Familiarity with PACE program regulations and guidelines required.
Skills & Attributes: Excellent interpersonal skills with the ability to build rapport with patients, families, and staff, with adaptability to work and travel across multiple care settings.
